Two Years of Specialized, Collaborative Care

Rezenity Health recently celebrated our second anniversary, and we are grateful for the opportunity to continue growing alongside the individuals, families, and healthcare professionals who trust us with their care! Over the past two years, our practice has more than doubled in size, allowing us to serve an increasing number of people while strengthening relationships with referral partners across the community.

 Even as we have expanded, our priorities have remained clear: timely access, high-quality care, and meaningful collaboration with the broader healthcare team.

 

At Rezenity Health, we believe that mental health care is most effective when it is connected to the rest of a person’s healthcare experience. Too often, when patients are referred to a community mental health provider, communication stops. In today's newsletter, we review our unique approach to collaboration, and why we prioritize continuity with the entire care team.


Our Pillars of Collaboration 

Collaboration begins before a first appointment is ever scheduled. When someone contacts Rezenity Health, we make it a priority to identify the referral source whenever possible. This helps us to close the communication loop with the healthcare provider, school professional, or community partner who helped connect that person to care.

 

When another healthcare provider initiates a referral, we notify them to either confirm that the individual has been scheduled for an appointment, or that our office was unable to reach them after multiple outreach attempts. These updates may seem simple, but they help other team members know if the next step in care is underway and whether additional follow-up may be needed.

 

With patient consent, we also share relevant clinical information with other members of the care team. This can include intake and discharge notes, as well as additional clinical insights obtained along the way when relevant. For example, a history of trauma may be important for a physical therapist to understand before beginning certain types of treatment. A change in eating patterns or body image concerns may be relevant to a physician or psychiatric prescriber. In these moments, communication helps the care team respond more thoughtfully, reduce avoidable misunderstandings, and support the patient with greater sensitivity.

 

Our goal is not simply to exchange paperwork, but to help each clinician understand the full picture of the person’s needs, strengths, and treatment plan.


Collaborative Care: Benefit for Patients and Providers

When clinicians communicate well, care feels more seamless. Patients are less likely to feel responsible for carrying messages from one provider to another, and providers are less likely to make decisions without the context they need. This kind of coordination helps reduce the silo that often separates mental healthcare from physical healthcare.

 

Collaborative communication also reinforces an important message: mental health and physical health are deeply connected. Emotional stress, trauma, anxiety, depression, sleep, pain, medical diagnoses, medication changes, and family systems can all influence one another.

 

The Challenges of Doing This Well

Meaningful collaboration takes time and effort. Our office staff tracks referral sources, documents outreach, coordinates releases of information, and ensures that updates are sent appropriately. Our clinicians also dedicate time within busy schedules to connect with other professionals, discuss concerns, and share insights that may influence care.

 

These efforts are not quick or easy. They require systems, follow-through, and a shared commitment to communication. However, we believe this investment is essential to providing high-quality mental healthcare in a community setting.
